An electronic health record (EHR) is a digital record of a patient's medical and health information. An EHR is similar to an electronic medical record (EMR) but includes additional health-related information about the person, beyond that which is included in his or her medical record. This electronic system of record keeping is intended to replace paper medical charts and health histories. Using an electronic record allow medical service providers to update patient care records confidentially and in real time. Electronic health records may be accessed by a patient's entire health care team, permitting better coordination of diagnosis and treatment.

An electronic health record usually includes a patient's medical history and other health information. By maintaining the information in electronic form, the records may be easily updated and access by the patient's health care providers. Ideally, the use of EHRs will increase the efficiency of health care provision. Because of the benefits associated with the use of electronic health records, in 2009 the U.S. government implemented an incentive plan for health care providers to begin tracking and recording individual patient records in electronic form.
