A personal wellness report, within the context of occupational health and safety, is the result or score of a wellness program evaluation. The personal wellness report will include the score and scoring system of the evaluation marking areas of focus, areas of improvement, ranges, and test explanations. Personal wellness report can also be referred as a wellness score.

A personal wellness report is generally produced after the completion of a health risk appraisal (HRA). The personal wellness report will include a series of parameters that will let the participant evaluate the general assessment of their current health practices. The report will highlight areas of improvement and include a series of recommendations in order to improve and manage individual health related issues.
